ABDM Milestones M1, M2 and M3 Explained for Hospitals
A plain-language guide to ABDM integration milestones: M1 (ABHA create and verify), M2 (hospital as Health Information Provider) and M3 (Health Information User), and the order to do them.
Quick answer
M1, ABHA: create and verify the patient's ABHA during registration.
M2, HIP: your hospital links its records to the patient's ABHA and shares them when the patient consents.
M3, HIU: your doctors request and view records from other facilities, with patient consent.
The registration desk creates a new ABHA with Aadhaar or mobile OTP, or verifies an existing one, and saves it on the patient record. This is the step patients notice, so explain the benefits of ABHA at the desk.
M2: Hospital as Health Information Provider
After M1, your software links visits, reports and prescriptions to the patient's ABHA. When another facility or the patient's health records app asks for them and the patient approves, your system sends them in the standard health data format.

M3: Hospital as Health Information User
Your doctors raise a consent request for a patient's records from other facilities. Once the patient approves, the records arrive in your software for the approved period only.
Recommended order
HFR for the facility and HPR for doctors
M1 in the sandbox, then production
M2, once ABHA is routinely captured at registration
M3, when doctors need outside records regularly
Choosing software
Ask any vendor which milestones they support today in production, not only in the sandbox. SmartHospital India includes M1 (ABHA create and verify) inside patient registration. For the wider picture, read our ABHA and ABDM integration guide.

Frequently Asked Questions
What are ABDM milestones M1, M2 and M3?
They are the stages of connecting hospital software to ABDM. M1 covers ABHA create and verify, M2 lets the hospital act as a Health Information Provider (HIP) that shares records with consent, and M3 lets it act as a Health Information User (HIU) that requests records from others.
Which ABDM milestone should a hospital start with?
Start with M1. It needs the least change at the front desk and is the base that M2 and M3 build on.
Does each milestone need separate testing?
Yes. Each milestone is built and tested in the ABDM sandbox before production access is granted for it.
