    What a valid prescription must contain in India

    A prescription is accepted by a pharmacist when it identifies the prescribing doctor, the patient and the drugs clearly. Every field below is already built into this generator, so the printed A4 sheet follows the standard Indian prescription format.

    Doctor identity

    Full name, qualification (MBBS, MD, MS, BAMS, BHMS) and the state medical council registration number.

    Clinic or hospital header

    Clinic name, address and contact number so a pharmacist can verify the prescription.

    Patient details

    Name, age, sex and, where relevant, weight — mandatory for paediatric and dose-sensitive drugs.

    Date of consultation

    Every prescription is dated; most retail pharmacies dispense against a prescription issued recently.

    Rx symbol and drug list

    Drug name (generic preferred), strength, dosage frequency, duration and instructions such as after food.

    Signature

    The doctor's signature or a valid digital signature, plus advice and the follow-up date.
