    What a discharge summary must contain in India

    A discharge summary is the handover document between the hospital, the patient and the next treating doctor or insurer. Every field below is already built into this generator, so the printed A4 sheet follows the standard Indian hospital format.

    Hospital header

    Hospital name, address and contact number, so any other doctor or insurer can verify the document.

    Patient and admission identity

    Patient name, age, sex, IPD number, ward and bed, date of admission and date of discharge.

    Diagnosis

    Provisional diagnosis at admission and the final diagnosis at discharge, written in full rather than as abbreviations.

    Clinical course and treatment

    Presenting complaints, key investigations, procedures or surgery performed and treatment given during the stay.

    Common discharge summary mistakes to avoid

    These six errors cause most insurance claim queries and follow-up confusion in Indian hospitals. The generator on this page is laid out to prevent each of them.

    Vague final diagnosis

    Writing only 'fever' or 'pain abdomen' instead of the confirmed diagnosis is the top reason for TPA claim queries. Write the diagnosis with site, side and stage where relevant.

    Missing procedure and date

    If a surgery or procedure was done, name it with the date and the operating surgeon. Insurers match this against the OT notes and the final bill.

    Unclear discharge medication

    Use one row per medicine with strength, frequency in the 1-0-1 format and duration in days, so the patient and the next doctor read it the same way.

    No follow-up date or danger signs

    Always print the follow-up OPD date and the danger signs that need an immediate return. It reduces avoidable readmissions and medico-legal disputes.

    No consultant signature

    An unsigned discharge summary is not accepted for claims. Print the consultant's name, qualification and registration number below the signature line.
